Abstract

Hypertension is one of the leading non-communicable diseases (NCDs) in Indonesia and the primary cause of cardiovascular complications such as stroke and coronary heart disease. Despite national programs, hypertension control remains low due to poor awareness, low health literacy, and treatment non-adherence. This literature review aims to analyze issues and trends in community-based hypertension management using three recent studies (2024 2025). The method used a systematic review guided by the PRISMA framework through Google Scholar and Garuda. Results show that community-based interventions—including cadre empowerment, early detection, interactive education, and culturally adapted health promotion—significantly improve adherence, knowledge, and blood pressure control. Key issues include the high hidden burden of hypertension, low adherence to medication, lack of regular monitoring, and geographic barriers in rural and coastal areas. Current trends reveal a shift toward participatory and collaborative approaches with community nurses as facilitators. Community-based interventions are promising strategies for reducing the national burden of hypertension.
